Instructions

  1. Please choose the following documents:
    1. Annual Report Form
  2. Read the opening memorandum and instructions carefully before you electronically complete both forms.
  3. Once form is completed, save the form to your computer by selecting the disk image.
  4. On this page below, input all relevant information.
  5. Select the "Choose File" or "Browse..." button.
  6. Select the saved form.
  7. Select “Open”
  8. Select “Upload File”
  9. Print the Affidavit Page (Page 14 of 14)
    1. Sign the Affidavit, and have it notarized by a Notary Public.
    2. Mail the notarized Affidavit together with the surety bond requirements and renewal fee of three hundred twenty five dollars ($325) for each registered location, to the Department of Financial Institutions, on or before June 1, 2014. Please also remit all outstanding examination fees.
  10. Verify that the following message is displayed: Thank you. Your file uploaded successfully.
