Instructions

  1. Please click the following link to the Check Cashing License Renewal Application and Annual Report Form: CC Renewal Application
  2. Read the opening memorandum carefully before you electronically complete both forms.
  3. Save the form as "Full Company's Name" to your computer by selecting the disk image.
  4. On this page, select the “Choose File” or "Browse..." button.
  5. Select saved form.
  6. Select “Open”
  7. Select “Upload File”
  8. Print the Affidavit Page (Page 8 of 12)
    1. Sign the Affidavit, and have it notarized by a Notary Public.
    2. Mail the notarized Affidavit together with the renewal fee of five-hundred dollars ($500) for each licensed location, to the Department of Financial Institutions, on or before September 1, 2014. Under Tennessee Code Annotated § 45-18-109(d), each licensee who fails to remit the renewal fee, renewal application and annual report by September 1, will incur a late fee of fifty dollars ($50) per day until these requirement are met. Please also remit all outstanding examination fees.
  9. Verify that the following message is displayed: Thank you. Your file uploaded successfully.
